Practice Policies
I consider it a privilege that you have chosen to treat with me at Resilient Growth and Wellness, PLLC, and I am committed to providing you with the best care possible. Your clear understanding of our Practice Policies is important to our professional relationship. For your convenience, this document covers the policies and procedures of the Practice. If you need further information or assistance about any of these policies, please let us know.
Cancellation/No Show Policy. I understand that there are times when you must miss an appointment due to illness, emergencies or obligations for work or family. However, when a patient does not show up, another patient loses the opportunity to be seen. If you need to cancel or reschedule an appointment, you must do so at least 48 hours in advance. If a patient does not notify the office of a request to reschedule or cancel at least 48 hours in advance they will be charged a cancellation fee of 50% of the appointment fee. If a patient does not cancel and does not show for their appointment, they will be charged the full appointment fee.
Late Arrivals. If you arrive more than 10 minutes late for your appointment, it will be assumed that you are a “no-show” and the appointment will be cancelled by your provider. If you are running late, please notify the Practice ASAP via telephone (508-286-8307) or via the patient portal. Arrivals more than 15 minutes late will require rescheduling and a late cancellation fee may apply.
If you accumulate multiple late cancellations or no-shows, or if you have 3 late arrivals, the Practice reserves the right to discharge you from the Practice or request prepayment to schedule future appointments. The prepayment will be credited to the appointment fee at the time of your appointment, and in the event of a no show, late cancellation or late arrival, the prepayment may be applied to Cancellation Fees, and will NOT be refunded.
Emergency Exceptions. We understand that emergencies happen. If you experience an unexpected situation, please contact our office as soon as possible to discuss your circumstances. In the event of a genuine emergency, the Practice will consider waiving the Cancellation Fee.
How to Cancel or Reschedule. To cancel or reschedule an appointment, please use the secure patient portal or call the practice at 508-286-8307. Do not send notify us via email.
Payments. Resilient Growth and Wellness, PLLC is a fee-for-service practice and does not accept any medical insurance. Payment is due at the time of your visit. Karin Rondeau, FNP (“Practitioner”) is considered an “out-of-network” provider for all insurers. The office cannot assist you with claim resolution. Patients are financially responsible for all services they receive at the Practice.
By signing below, Patient acknowledges that (i) the Services provided at Resilient Growth and Wellness are not covered by insurance, Medicare, Medicaid and/or any other third-party payor, and (ii) neither the Practice nor Practitioner participate in any health insurance or HMO plans or panels and has opted out of Medicare.
Resilient Growth and Wellness accepts payments through its electronic payment system. If patients use their flex spending or HSA, a formal health form can be given to the patient to submit for reimbursement after the payment has cleared (upon request only).
Collection Costs. In the event that a patient’s account is placed in collection status, an administrative fee will be added to the account balance, and the patient will be responsible for all attorneys’ fees and costs incurred in collecting any outstanding balance.
Laboratory Services. Resilient Growth and Wellness may recommend that you undergo laboratory tests. When possible, these tests will be recommended to be obtained through Quest. Patients are free to obtain such tests from any lab and provider they choose. There may be additional out-of-pocket costs for this lab work, which patients are responsible for. Lab fees are not included in your cost of treatment at the practice. Resilient Growth and Wellness is not responsible for the cost or payment of any bills incurred for labs or tests that are performed, and it is recommended that patients check with their insurance carrier regarding coverage prior to having any lab work done.
Any lab testing that is requested by Resilient Growth and Wellness is not intended to diagnose a disease. The testing will help you practitioner make the appropriate recommendations. This testing is not intended to replace testing recommended by your primary care provider or any other medical provider you treat with.
Privacy. Practitioner and the Practice are required by law to maintain the privacy of protected health information. At the Practice your privacy will be protected and your medical records will not be shared without permission, except as required by law, or as requested by patient or patient representative through legally designated acknowledgments. The Practice reserves the right to amend these Policies and will inform you of any changes.
